弟子某甲等 稽首歸命禮 遍虛空法界 十方諸如來 瑜伽總持教 諸大菩薩眾 及禮菩提心 能滿福智聚 令得無上覺 是故稽首禮
禮佛真言曰。
唵薩嚩怛他孽多(引)跛娜滿那喃迦嚧彌
次應運心供養。
弟子某甲等 十方一切剎 所有諸供養 花鬘燈涂香 飲食幢幡蓋 誠心我奉獻 諸佛大菩薩 及諸賢聖等 我今至心禮
普供養虛空藏真言曰。

次應懺悔。

次應懺悔。
弟子某甲等,今對一切佛, 諸大菩薩眾,自從過去世, 無始流轉中,乃至於今日, 愚迷真如性,起虛妄分別, 貪瞋(chēn)癡不善,三業諸煩惱, 及以隨煩惱,違犯他勝罪, 及餘罪愆(qiān)等,譭謗佛法僧, 侵奪三寶物,廣作無間罪, 無量無邊劫(jié),不可憶知數, 自作教他作,見聞及隨喜, 復依勝義諦(dì),真實微妙理, 聖慧眼觀察,前後中三際(jì), 彼皆無所得。

'Receiving the Bodhicitta Precepts Ritual', one scroll

Disciples, such as so-and-so, prostrate and take refuge in: The Buddhas of the ten directions throughout the boundless Dharma Realm, The Yoga Total Retention teachings, all the great Bodhisattvas, And pay homage to the Bodhicitta (the mind of enlightenment), which can fulfill the accumulation of merit and wisdom, Enabling us to attain unsurpassed enlightenment, therefore, we prostrate and pay homage.

Next, one should contemplate and make offerings.
Disciples, such as so-and-so, in all lands of the ten directions, All the offerings that exist, flowers, garlands, lamps, scented unguents, Food, banners, canopies, with sincere hearts, we offer them, To all Buddhas, great Bodhisattvas, and all virtuous sages, We now pay homage with utmost sincerity.

Om Gagana Sambhava Vajra Hoḥ
Next, one should repent.
Disciples, such as so-and-so, now before all the Buddhas, All the great Bodhisattvas, from the past lives, In the beginningless cycle of rebirth, up to this day, Deluded about the nature of Suchness (Tathata), arising from false discriminations, Greed, hatred, ignorance, unwholesome deeds, the three karmas and all afflictions, And the secondary afflictions, violating the Parajika (defeats) offenses, And other transgressions, slandering the Buddha, Dharma, and Sangha, Seizing the property of the Three Jewels, extensively committing the Avici (uninterrupted) hell karma, For immeasurable and boundless kalpas, an uncountable number, Done by oneself, taught to others to do, seen, heard, and rejoiced in, Relying on the ultimate truth, the true and subtle principle, Observed by the sacred wisdom eye, the three periods of past, present, and future, All of them are unobtainable.
自心造分別 虛妄不實故 以為慧方便 平等如虛空 我悉皆懺悔 誓不敢覆藏 從今懺已后 永斷不復作 乃至成正覺 終更不違犯 唯愿十方佛 一切菩薩眾 哀愍加護我 令我罪障滅 是故至心禮

【English Translation】 English version The mind itself creates distinctions, Because these distinctions are false and unreal, I mistakenly take them as wisdom and skillful means. True equality is like the void, all-pervading. For these errors, I completely repent. I vow not to conceal my faults anymore, and from today after this repentance, I will forever cut off these errors and not repeat them, until I attain perfect enlightenment (Anuttara-samyak-sambodhi). I will ultimately not violate these vows again. I earnestly request the Buddhas of the ten directions, and all the Bodhisattvas, to have compassion and protect me, so that my karmic debts and obstacles may be eliminated. Therefore, I sincerely prostrate.
The true mantra for repenting and eliminating sins says:
Om Sarva Papa Dahana Vajraya Svaha
Next, one should take the Three Refuges.
Disciples, so-and-so, from today onwards, take refuge in all the Tathagatas, the Buddhas of the Five Wisdoms and Trikaya (Three Bodies). take refuge in the Vajrayana, the Dharmadhatu (True Nature of Reality). take refuge in the irreversible Great Compassion Bodhisattva Sangha. Having taken refuge in the Three Jewels, I will ultimately not take refuge in the selfish and heretical paths. I now sincerely prostrate.
The mantra for taking the Three Refuges says:

Next, one should receive the Bodhicitta Vows.
Disciples, so-and-so, before all the Buddhas and Bodhisattvas, from today onwards, until I attain perfect enlightenment (Anuttara-samyak-sambodhi), I vow to generate the Bodhicitta (Awakening Mind).
I vow to liberate limitless sentient beings; I vow to accumulate limitless merit and wisdom.
I vow to learn limitless Buddha-dharma; I vow to serve limitless Tathagatas.
I vow to achieve unsurpassed Bodhi (Anuttara-samyak-sambodhi).
The awakened mind that is now generated is free from all inherent nature and phenomena, free from the five skandhas (aggregates), twelve ayatanas (sense bases), and eighteen dhatus (elements), as well as the grasping of the grasper and the grasped. All dharmas are without a 'self'; they are equal like the void. The mind itself is originally unborn, because its emptiness is perfectly tranquil. Just as all the Buddhas and Bodhisattvas generate the Great Bodhicitta (Mahabodhi-citta), I now generate the Bodhicitta in the same way. Therefore, I sincerely prostrate.

教受發菩提心戒懺悔文
弟子某甲等。歸命十方一切諸佛諸大菩薩大菩提心為大導師。能令我等離諸惡趣。能示人天入大涅槃。是故我今至心頂禮。
弟子某甲等。十方世界所有一切最勝上妙。香花幡蓋種種供養。奉獻一切諸佛菩薩。至心頂禮。
弟子某甲等。自從過去無始已來。乃至今日。貪瞋癡等種種煩惱。及忿恨等諸隨煩惱。惱亂身心。廣作一切身業不善殺盜邪淫。口業不善妄言綺語惡口兩舌。意業不善貪瞋邪見。種種煩惱。無始相續纏染其心。令身口意造罪無量。或殺父母殺阿羅漢。出佛身血破和合僧。譭謗三寶打縛眾生。破齋破戒飲酒食肉。及食五辛。如是等罪無量無邊。不可憶知。今日誠心發露懺悔。一懺已后。永斷相續更不敢造。唯愿十方一切諸佛諸大菩薩。加持護念。能令我等罪障銷滅。

【English Translation】 English version Teaching the Repentance Text for Receiving the Bodhicitta Vows
Disciple so-and-so, takes refuge in all Buddhas and great Bodhisattvas of the ten directions, taking the great Bodhicitta (Great mind of enlightenment) as our great guide. It can lead us away from all evil destinies and show humans and devas the way to enter Great Nirvana (state of liberation). Therefore, we now sincerely prostrate ourselves.
Disciple so-and-so, offers the most supreme and wonderful incense, flowers, banners, canopies, and various offerings from all the ten directions to all Buddhas and Bodhisattvas, sincerely prostrating ourselves.
Disciple so-and-so, since the beginningless past until today, due to various afflictions such as greed, anger, and ignorance, as well as various secondary afflictions such as resentment and hatred, we have troubled our body and mind, extensively creating all unwholesome bodily actions such as killing, stealing, and sexual misconduct; unwholesome verbal actions such as false speech, embellished speech, harsh speech, and divisive speech; and unwholesome mental actions such as greed, anger, and wrong views. Various afflictions have continuously entangled and defiled our minds since beginningless time, causing our body, speech, and mind to create immeasurable sins. We may have killed our parents, killed an Arhat (one who has attained Nirvana), shed the blood of a Buddha, disrupted the harmony of the Sangha (Buddhist monastic community), slandered the Three Jewels (Buddha, Dharma, Sangha), beaten and bound sentient beings, broken fasts and precepts, drunk alcohol, eaten meat, and consumed the five pungent roots (garlic, onions, leeks, chives, and asafoetida). Such sins are immeasurable and uncountable. Today, we sincerely confess and repent, vowing to forever cease such actions after this repentance and never dare to commit them again. May all Buddhas and great Bodhisattvas of the ten directions bless and protect us, enabling us to eradicate our karmic obstacles.
Disciple so-and-so, from this life until we sit at the Bodhi-tree (tree of enlightenment) in the future, in the meantime, we take refuge in the Tathagata's (Buddha's) unsurpassed Three Bodies (Dharmakaya, Sambhogakaya, Nirmanakaya), take refuge in the vast and extensive Mahayana Dharma Treasury, and take refuge in the Sangha of all non-retreating Bodhisattvas. Having taken refuge in the Buddha, having taken refuge in the Dharma, having taken refuge in the Sangha, from now on, we will no longer take refuge in the Two Vehicles (Shravakayana and Pratyekabuddhayana) or external paths. May all Buddhas of the ten directions witness us, sincerely prostrating ourselves.
Disciple so-and-so, starting from this life until we sit at the Bodhi-tree in the future, in the meantime, we vow to generate the unsurpassed Bodhicitta.
Sentient beings are boundless, we vow to liberate them all; blessings and wisdom are boundless, we vow to accumulate them all; Dharma gates are boundless, we vow to learn them all; Tathagatas are boundless, we vow to

May I attain skillful wisdom and fully embody the perfect heart of Samantabhadra (普賢). I sincerely wish that all Buddhas and great Bodhisattvas in the ten directions will bear witness to us. With utmost sincerity, we prostrate ourselves.
