大興善寺三藏沙門大廣智不空奉 詔譯
佛一時在維耶離樂音樹下。與大菩薩三萬六千比丘八萬人俱。及十二神王並諸眷屬天龍八部大神王。在如是等大眾會中。說此法已。大眾皆聞無不歡喜。是名即為結愿神咒即薄伽梵。說咒曰。
南謨薄伽筏帝鞞殺社窶嚕蔽琉璃缽喇婆喝啰阇耶怛他揭多怛侄他唵鞞殺逝鞞殺逝鞞殺社三沒揭帝娑婆訶
若有受持此真言。能拔身中過去生死一切重罪。不復經歷三途。免離九橫超越眾苦。十方世界隨處安樂。自在無礙法應如是。若善男子善女人等。受持讀誦是真言者。日夜精勤香湯洗浴著新凈衣。持諸禁戒如法。誦滿真言十萬遍已。就清凈處如法治地。以凈土筑令平。以凈牛糞和檀香涂圓壇。以種種雜寶莊嚴壇。安中心一藥師如來像如來左手令執藥器。亦名無價珠。右手令作結三界印。一著袈裟結跏趺坐。令安蓮華臺。臺下十二神將。八萬四千眷屬上首。令 又令須蓮臺。如來威光中令住日光月光二菩薩。如是壇四方周匝五色。近前安置二閼伽器商佉瓦器。隨意受用奉獻承事尊像。像前唸誦四十九日間。若三七日。畫像壇四角安置賢

【English Translation】 English version At one time, the Buddha was under the Tree of Joyful Sound in Vaishali (an ancient Indian city). He was with 36,000 great Bodhisattvas, 80,000 Bhikshus (monks), as well as the Twelve Yaksha Generals and their retinues, the Eight Classes of Gods and Dragons, and other great divine kings. In such a great assembly, the Buddha spoke this Dharma, and all those present heard it and were filled with joy. This is called the Concluding Vow Dharani, which is the mantra spoken by the Bhagavan (World-Honored One): Namo Bhagavate: Homage to the Bhagavan Bhaisajya-guru: Medicine Master Vaidurya-prabharajaya: Lapis Lazuli Light King Tathagataya: Thus Come One Tadyatha: Thus it is Om: The primordial sound Bhaisajye: Medicine Bhaisajye: Medicine Bhaisajya: Medicine Samudgate: Universal liberation Svaha: Accomplishment If anyone upholds this true mantra, they can eradicate all the heavy sins of past lives and deaths, no longer experience the three lower realms (hell, hungry ghosts, animals), avoid the nine untimely deaths, and transcend all suffering. In the ten directions of the world, they can find peace and happiness wherever they are, and be free and unhindered. The Dharma should be practiced in this way. If good men and good women uphold and recite this true mantra, diligently day and night, bathe with fragrant water, wear new and clean clothes, uphold all the precepts, and recite the true mantra one hundred thousand times according to the Dharma, then they should prepare a clean place according to the Dharma, compact the ground with clean soil to make it level, and smear a round altar with clean cow dung mixed with sandalwood. Adorn the altar with various precious treasures. In the center of the altar, place an image of the Medicine Master Tathagata. The Tathagata's left hand holds a medicine bowl, also called the priceless pearl, and the right hand forms the Three Realms Mudra. He wears a kasaya (monk's robe), sits in the lotus position, and is placed on a lotus platform. Below the platform, place the Twelve Yaksha Generals and the leaders of the 84,000 retinues. There must also be a lotus platform. In the radiance of the Tathagata, place the Sunlight Bodhisattva and the Moonlight Bodhisattva. Such an altar should be decorated with five colors on all four sides. Place two argha vessels (water offering vessels) and a shankha vessel (conch shell) in front of the altar, for offering and serving the honored image. Recite in front of the Buddha image for forty-nine days, or twenty-one days. Place the Wise ones at the four corners of the painted altar.
瓶。如是晝夜七日內。誦咒數滿百千萬遍。所求從心無量獲果報。除不至心。法應如是。所有利益說不可盡。其餘功能窮劫不可說。

【English Translation】 English version: If done in this way, within seven days and nights, reciting the mantra a million times, all wishes will be fulfilled and immeasurable rewards will be obtained, unless one is insincere. This Dharma should be practiced in this way. All the benefits are inexhaustible to describe, and the remaining functions cannot be fully described even in countless kalpas.
Next, the fundamental mudra of Bhaisajya-guru Tathagata (Medicine Buddha) is explained. Cross the eight fingers below the index fingers of the left and right hands inward into the palms. Move the two thumbs back and forth. The mantra is:
Om chandha ghiri svaha (This is called the fundamental mudra)
Next, the Argha mudra is explained. Hold the vessel with both palms and place it on the head. The mantra is:
Namo samanta buddhanam kakka mum sama sama svaha

如果患有身體支節疼痛,唸誦咒語加持湯水一百零八遍,洗浴就能得到消除痊癒。
如果有人每天早上,用手捧一掬水,唸誦咒語七遍后飲用,在身上所有的惡報都能消滅,更何況是沒有各種災厄的人呢。以及各種身、口、意三業的痛苦也能消除痊癒,並且能夠壽命長遠。
如果唸誦咒語加持飲食,一切毒物都不能造成損害。
如果見到惡人以及有怨仇的人,應當唸誦這個咒語,所有怨家生起噁心的人,都會被降伏,噁心立即消滅,轉而以慈悲心相待。在有恐怖的地方,應當收攝心神唸誦。
如果想要睡覺時,應當唸誦這個咒語一百零八遍,就能得到好夢,善於知曉吉兇。
如果有人患有瘧疾,持誦這個咒語的人看著患瘧疾的人,懇切地念誦這個咒語一千零八遍,他的疾病就能消除痊癒。

【English Translation】 English version A wheel. Place it before the image. Make a rope with five-colored threads. Seal it to a pillar with a stamp. Tie forty-nine knots around that person's body. Then recite the 'Bhaisajyaguru Sutra' (藥師經) [Medicine Buddha Sutra] forty-nine times. All sins and obstacles will be dissolved, lifespan will be extended, and one will be secure from unexpected suffering. Diseases caused by ghosts and spirits will also be immediately cured.
If you wish to subdue an evil person, make a mudra (印) [hand gesture], recite the mantra one hundred and eight times with an angry mind, and point it remotely at all heretics to destroy them.
If someone suffers from heart disease, bless green sandalwood and apply it to the heart to cure it.
If someone suffers from a headache, bless cinnamon bark twenty times and take it to cure it.
If someone suffers from swelling and jaundice, bless turmeric one hundred and eight times and apply it to cure it.
If someone wants to get away from enemies, bless bitter neem seeds one hundred and eight times, burn one seed with each recitation, and throw them into the fire. That person will then go far away.
If you suffer from pain in the body's joints, bless hot water one hundred and eight times with the mantra, and bathe in it to be cured.
If someone every morning takes a handful of water, blesses it seven times with the mantra, and drinks it, all the bad karma on their body will be eliminated. How much more so for those without various calamities. And the suffering of the three karmas (身口意) [body, speech, and mind] will also be cured, and one will gain longevity.
If you bless food and drink with the mantra, all poisons will be unable to harm you.
If you see an evil person or an enemy, you should recite this mantra. All enemies who harbor evil intentions will be subdued, their evil intentions will immediately disappear, and they will treat you with compassion. In places of fear, you should concentrate your mind and recite the mantra.
If you want to sleep, you should recite this mantra one hundred and eight times, and you will have good dreams and be able to know good and bad omens.
If someone is suffering from malaria, the person holding this mantra should look at the person suffering from malaria and sincerely recite this mantra one thousand and eight times, and their illness will be cured.

【English Translation】 English version: This is from the Dharma Drum Platform of Kozan-ji (高山寺), based on a version written and proofread at Ninna-ji (仁和寺). Nozawa Suetsugu Jogyo (野澤末資淨嚴), age thirty-six.
In the fifth month of the Xin Chou (辛丑) year of Tenmei 1 (天明元年), copied from the version of Kakuen Sojo (覺遠僧正). Jinin (慈忍), recorded in the Xin You (辛酉) year of Kyowa 1 (享和元年). Requested to use the above version as a basis to interpret it using Japanese characters (訓國字). This is the 'Yakushi Nenshu Ki' (藥師念誦軌, Medicine Buddha Recitation Ritual). It was already published during the Genbun (元文) era. However, that version was incomplete, missing one page. Therefore, it has been re-collated for publication. Written by Kaido-shi (快道志), a Shramana (沙門) residing at Hasedera (長谷寺) in Buzan (豐山).
