藥師如來唸誦儀軌一卷
大興善寺三藏沙門大廣智不空奉 詔譯
佛一時在維那離音樂樹下。與三萬六千比丘人俱。及十二神王。並諸眷屬天龍八部大神王。在如是等大眾會中。說是法已。大眾皆聞無不歡喜。是即名為結愿神咒。即薄伽梵說咒曰。
南謨薄伽筏帝鞞殺社寠嚕蔽琉璃缽唎婆喝啰阇耶怛他揭多怛侄他唵鞞殺逝鞞殺社三沒揭帝婆婆呵
若有受持此真言。能拔身中過去生死一切重罪。不復經歷三塗。免離九橫超越眾苦。
根本印真言。以左右手頭指以下八指。反叉入于掌。以二大指來去咒曰。
唵戰馱祇哩娑婆呵
次結護贊。
薩縛勃馱曩薩縛爾曩(入)迦盧拏怛么(二合平)迦訶羅(引)睹迷(存念於我)俱嚕吠女哩也(二合)入縛羅(引)怛他誐多麼訶捺耶(成就義)曩謨悉羯羅(歸命頂禮義)

佛陀一時在維那離音樂樹下。與三萬六千比丘（bhiksu，佛教出家男眾）一同。以及十二神王，和他們的眷屬天龍八部大神王。在這樣的一個大眾集會中。說完此法之後，大眾都聽聞了，沒有不歡喜的。這就是所謂的結愿神咒。即薄伽梵（Bhagavan，世尊）說咒語道：
南謨薄伽筏帝（Namo Bhagavate） 鞞殺社窶嚕（Bhaisajya-guru） 蔽琉璃缽唎婆喝啰阇耶（Vaidurya-prabharajaya） 怛他揭多耶（Tathagataya） 怛侄他（Tadyatha） 唵（Om） 鞞殺逝（Bhaisajye） 鞞殺社（Bhaisajye） 三沒揭帝（Samudgate） 婆婆呵（Svaha）
若有人受持這個真言，能夠拔出身中過去生死的一切重罪，不再經歷三塗（地獄、餓鬼、畜生），免離九橫（九種意外死亡），超越各種痛苦。
根本印真言。用左右手頭指以下的八指，反叉入于掌中。用兩個大拇指來回移動，唸誦咒語道：
唵（Om） 戰馱祇哩（Canda-ghri） 娑婆呵（Svaha）
其次結護贊。
薩縛勃馱曩（Sarva-buddhana） 薩縛爾曩（Sarva-jna） 迦盧拏怛么（Karuna-atma） 迦訶羅（Kalaha） 睹迷（Dume） 俱嚕吠女哩也（Kurve-virya） 入縛羅（Jvala） 怛他誐多麼訶捺耶（Tathagata-mahadaya）（成就義） 曩謨悉羯羅（Namo-styakara）（歸命頂禮義）

【English Translation】 English version The N唸誦 Ritual of Bhaisajyaguru Tathagata, One Fascicle
Translated by Tripitaka Master Da Guang Zhi Bu Kong of Da Xing Shan Temple under Imperial Decree
At one time, the Buddha was beneath the Vinaliya Music Tree, together with thirty-six thousand bhiksus (Buddhist monks), as well as the Twelve Divine Kings, along with their retinue of gods, dragons, the eight classes of beings, and great divine kings. Having expounded this Dharma in such a great assembly, all who heard it rejoiced without exception. This is known as the vow-fulfilling dharani. Thereupon, the Bhagavan (the Blessed One) spoke the dharani, saying:
Namo Bhagavate Bhaisajya-guru Vaidurya-prabharajaya Tathagataya Tadyatha Om Bhaisajye Bhaisajye Samudgate Svaha
If one upholds this true mantra, one can eradicate all the heavy sins of past lives and deaths from one's body, no longer experience the three lower realms (hell, hungry ghosts, animals), avoid the nine untimely deaths, and transcend all suffering.
The fundamental mudra and mantra. With the eight fingers from the index fingers of both hands, interlock them reversely within the palms. Move the two thumbs back and forth, chanting the mantra:
Om Canda-ghri Svaha
Next, the protective praise.
Sarva-buddhana Sarva-jna Karuna-atma Kalaha Dume Kurve-virya Jvala Tathagata-mahadaya (meaning accomplishment) Namo-styakara (meaning to pay homage and prostrate)
