何耶揭唎婆像法
複次取凈褺不得截割。請一畫師最功能。勿還其價。香湯洗浴著新凈衣與受八戒。日日如是。于清凈處作一水壇。縱廣四肘(水壇之法如餘部說)咒師護身結界畢已。于其壇中燒種種香。散種種華供養已訖。於此壇內畫作菩薩。其身長佛一磔(長短正當以人一肘)總有四面。中菩薩面極令端正。作慈悲顏顏色赤白頭髮純青。左邊一面作大瞋怒黑色之面。狗牙上出頭發微豎如火焰色。右邊一面作大笑顏。赤白端正似菩薩面。頭髮純青三面頭上。各戴天冠及著耳珰。其天冠上有一化佛結跏趺坐。中面頂上作碧馬頭仍令合口。菩薩頸下著寶瓔珞。項背圓光數重色作。左手屈臂手當乳前把紅蓮花。其蓮花與菩薩頭齊正臨左膊。其華臺上作一化佛。著緋袈裟結跏趺坐。項背有光。右手仰掌五指申。臂肘平屈。其手掌擎真陀摩尼(唐云如意寶)其珠團圓如作白色。赤色光炎圍繞其珠。于其右手正當珠下雨種種寶。其左膊上。著弊耶(二合)迦羅者摩(二合唐云虎皮)如著祇支。當右腋下掩皮結帶。更用虎皮縵其跨上。以外臂釧天衣裙等。皆如余處畫菩薩法。

【English Translation】 English version: Hayagriva Image Dharma
Furthermore, take a clean cloth that has not been cut. Invite a painter with the best skills, and do not pay him. Bathe him with fragrant water, dress him in new, clean clothes, and have him take the eight precepts. Do this every day. In a clean place, make a water altar, four cubits in length and width (the method for making the water altar is as described in other texts). After the mantra master has completed the protection and boundary rituals, burn various incenses and scatter various flowers in the altar as offerings. Then, within this altar, paint an image of the Bodhisattva. The height of the body should be one vidasti (the length should be based on a person's cubit), and there should be a total of four faces. The middle Bodhisattva face should be painted extremely dignified, with a compassionate expression, reddish-white in color, and pure blue hair. The face on the left should be painted as a greatly wrathful black face, with dog teeth protruding upwards and hair slightly raised, the color of flames. The face on the right should be painted with a great smiling expression, reddish-white, dignified, similar to the Bodhisattva's face, with pure blue hair. Each of the three faces wears a heavenly crown and earrings. On the heavenly crown is a manifested Buddha sitting in the lotus position. On the top of the middle face, paint a turquoise horse head, and keep its mouth closed. The Bodhisattva wears a jeweled necklace around his neck, and there are multiple layers of colored halos behind his neck. The left hand is bent at the elbow, with the hand in front of the chest, holding a red lotus flower. The lotus flower is level with the Bodhisattva's head, directly facing the left shoulder. On the lotus flower platform is a manifested Buddha, wearing a scarlet kasaya, sitting in the lotus position, with a halo behind his neck. The right hand is with the palm facing upwards, with the five fingers extended, and the elbow bent horizontally. The palm holds a Cintamani (translated as 'wish-fulfilling jewel' in the Tang dynasty), the jewel is round, white in color, and surrounded by reddish light flames. Directly below the jewel on the right hand, rains down various treasures. On the left shoulder, he wears a vyaghra-ajina (translated as 'tiger skin' in the Tang dynasty), like wearing a garment covering the chest and back. Cover the tiger skin under the right armpit and tie it with a belt, and then cover the tiger skin over his hips. As for the outer arm bracelets, heavenly clothing, etc., they are all like the methods for painting Bodhisattvas elsewhere.
如令菩薩端身正立紅蓮華上。空懸寶傘蓋菩薩頭上。其上空中畫作種種天音樂具。兩邊空中須陀會天舞蹈供養。畫此像時用香汁畫。皮膠不當用。叉以蘇木佉子(唐云蕓臺)搗以為末。極令微細用酪(作酪殘汁也)汁。和如和麨法。用何耶揭唎婆心咒。咒三七遍已。其病人空腹腹之。腹內所有一切毒蟲悉皆吐出。又畫作像法。取新瓦雜瓶未曾用。瓶染作青色。其瓦瓶上畫作阿耶揭唎婆觀世音像。其像身高如來一磔(等人一肘)畫作四個歡喜之面。左邊一面畫作黑色。眼精綠色狗牙上出。右邊一面畫作赤色。名吃咒面。當中前面作菩薩面。極令端正。畫作白色。離像頂上懸于空中。畫作青面。出口吐寶珠。其四頭上各戴寶冠。其寶冠上皆化佛坐。菩薩左手把于蓮華。屈肘向上捧在膊前。右臂垂下五指皆申。施無畏手。兩手腕上皆作寶釧。其像項下著寶瓔珞。身份莊嚴如余處說觀世音身莊嚴之法。其像立在寶蓮花上。作此像已。從白月十日起首食大麥乳糜。后十三日十四日十五日。三日三夜斷食。取娑迦比遮香乾陀那句利華咒此二物。一咒一投目前面上至八千遍。其面口中放出大光明。圍繞咒師身。已還入像口。于其口中出如意珠。即收取之珠已。即得十四千歲壽命。得七寶轉輪聖王。命終以後生安樂國。從觀世音足下而
坐。又有一法當乞食吃吃大麥乳糜誦七萬遍。一萬遍時毗那耶迦即得被縛。第二萬遍咒法成就。第三萬遍即得安善那摩那熾羅藥法成就。三日三夜不食。作法手把其藥。正咒藥時。藥中若現三種相者。即知藥成。三相者謂暖煙焰。即得成就安陀羅陀那。升空而去得成就持咒仙人。自在得入阿修羅宮。第四萬遍離地四指而行自在。第五萬遍則得隨意升空行。第六萬遍即得千歲活。咒師成就咒仙。人隨其所作任運皆成。一切所呼喚者皆來。第七萬遍咒師即得頭髮變作螺發成就。隨其所須應念即至。親近承事于觀世音。即知三世一切眾生死此生彼。即得成就。若又法若欲打西(去音)若(若耶反)者。何耶揭唎婆像前三日三夜斷食香摩壇上。種種香花。以凈飲食乳粥果等種種供養。搗娑阇羅娑香。以白蜜和為八千丸。咒一丸香一遍已。投火中燒。如是香盡。彼西惹眾尋即初見變為馬頭。更經少時。轉為何耶揭哩婆頭。咒師奇上向彼眾時。一切西惹眾悉皆散滅。又法先以大麥與牛吃已。取糞中麥洗干誠搗煮為乳糜。從白月一日食此乳糜。至十三日即斷不食。何耶揭哩婆像前。以涂香塗地散種種花。燒諸名香。種種飲食供養畢已。取迦比阇香搗為細末。水和為丸。數滿八千。取迦地羅木為柴燃火。取一香丸一咒一投。於火中燒。
如是乃至八千丸。盡其炭變為蘇跋那金黃。重百千兩。又法若欲吃他咒者。取己身中小許血已。和白芥子為八千丸。黑月十四日。一日不食。取佉陀羅木爐中燃火。取前白芥子丸。一咒一丸投火中燒。如是乃至芥子丸盡。一切諸咒悉皆吃盡(吃云外國語也。即同大唐破滅語訓。他者是外道也)又法但誦咒。行於曠野處及諸沙磧。所須飲食自然而出。又法日月蝕時。取凈牛蘇一兩新瓦缽盛。即把此缽仰看日月。咒缽中蘇乃至蘇暖。或煙火出。當爾之時即飲此蘇服此蘇已。隨欲詣處舉意即至。若常誦此咒。一切怖畏得無所畏。所有障難悉皆解脫。又法若患路陀瘡(患此瘡者悉皆死也)及諸毒蛇之所咬者(此蛇人見即死)或患健毗吒雞瘡。此等諸瘡咒黃土泥。滿一千遍。涂其瘡上即得除差。一切怖畏心念即得除。又法若人忽逢一切水難。心念即得不被沉溺。如是等法不可廣說。隨意即成。若造大曼荼羅者。不得行欲。

【English Translation】 English version: Thus, make eight thousand pills. Burn all the charcoal until it turns into Subarna (gold) color, each weighing one hundred thousand 'liang'. Another method: if you want to 'eat' other mantras ('eat' is a foreign word, equivalent to 'destroy' in Tang Chinese; 'other' refers to non-Buddhist paths), take a small amount of your own blood, mix it with white mustard seeds, and make eight thousand pills. On the fourteenth day of the dark month, do not eat for one day. Take a Khādira (a type of tree) wood stove and light a fire in it. Take the white mustard seed pills, and with each recitation of the mantra, throw one pill into the fire to burn. Continue until all the mustard seed pills are burned, and all mantras will be 'eaten'. Another method: simply recite the mantra and walk in the wilderness or deserts, and the necessary food and drink will naturally appear. Another method: during a solar or lunar eclipse, take one 'liang' of pure cow ghee and put it in a new earthenware pot. Then, hold the pot and look up at the sun and moon, chanting the mantra until the ghee becomes warm or smoke and fire appear. At that time, drink or consume the ghee, and then, wherever you wish to go, you will arrive instantly with a thought. If you constantly recite this mantra, all fears will be eliminated, and all obstacles will be liberated. Another method: if you suffer from a Rudra sore (those who suffer from this sore will die), or are bitten by a poisonous snake (this snake kills people upon sight), or suffer from a Jianpiṭaji sore, chant over yellow earth mud one thousand times and apply it to the sore, and it will be cured. All fears will be eliminated with a single thought. Such methods cannot be fully described; they can be accomplished at will. If you are constructing a large Mandala (sacred enclosure), you must abstain from sexual activity.
Hayagrīva (Horse-Headed King) Healing Mudra (Interlock the hands inward, raise the two middle fingers straight with the tips touching, bend the two thumbs, and use the two index fingers to bend and grasp the root of the thumb nails, with the backs of the index finger nails touching each other).

已上二印可在解禁刀印之次。若作此法先當縛禁毗那夜迦。當對賀耶揭哩婆像前。取后木長八指截。燒火焰出。取安悉香作八百丸。一丸一咒一投火中。乃至香盡。毗那夜迦。即自被縛咒曰。

又法咒袈裟誦一七遍。已結作一結。即得護身成就咒曰(歸命)。

【English Translation】 English version: The above two mudras can be used when lifting the restrictions of the knife mudra. If you want to perform this method, you should first bind and restrain Vinayaka (象頭神). In front of the Hayagriva (馬頭明王) image, take a piece of wood eight fingers long, cut it off, and burn it to produce flames. Take benzoin and make eight hundred balls. Recite a mantra for each ball and throw it into the fire until all the incense balls are burned. Vinayaka (象頭神) will then be bound. The mantra is: Namo Tadyatha (Homage, beginning of the mantra) (1), Chulu Chulu (2), Vi Chulu Chulu (3), Gata Ye Svaha (May this mantra be accomplished) (4), Maha Bha (Great) (去音) (5), Sarva Vighna Vinayakan (All obstacles, Vinayaka) (上音 6), Pata Bhamo Kuchih (去音二合 7), Bhuta (鬼) Ye Phat (8). If someone suffers from nocturnal emission, take a white thread and recite the mantra eight hundred times.
