若有沙門。若婆羅門諸善男子善女人等。欲受持菩薩法者。應作四肘壇。當覓勝地清凈之所掃灑清凈。以香水牛糞泥地。懸于種種雜色幡蓋寶鈴珮鏡。並諸金銀種種間錯嚴錺道場。其道場中立五色壇。縱廣四肘。先下白色次黃次赤次黑。而作四門。其壇中心作蓮華座。安置何耶揭利婆觀世音像。東門作華座安十一面菩薩。北門作蓮華座安八臂觀世音。南方安八龍王。難陀龍王。婆素難龍王。德叉迦龍王。羯固姹龍王。般摩龍王。摩訶般摩龍王。商佉波羅龍王。鳩利迦龍王(八龍唯供粳米乳糜。餘者以種種供養亦得)燃四十五燈。先喚八個龍王(用何耶揭唎婆身印來去)。

Hayagriva (Horse-headed Avalokiteśvara) Avalokiteśvara Bodhisattva Empowerment Mandala

If there are Shramanas (ascetics). Or Brahmanas (ancient Indian priestly class), virtuous men and virtuous women, etc., who wish to receive and uphold the Bodhisattva Dharma, they should construct a four-cubit mandala. They should seek a superior and pure place, sweep and purify it. Smear the ground with fragrant water and cow dung. Hang various colorful banners, canopies, jeweled bells, pendants, and mirrors. And adorn the Dharma hall with various gold and silver interspersions. In the center of the Dharma hall, erect a five-colored mandala, four cubits in length and width. First, apply white, then yellow, then red, then black, creating four gates. In the center of the mandala, create a lotus seat. Place the image of Hayagriva (Horse-headed) Avalokiteśvara. At the east gate, place a flower seat and install the Eleven-Faced Bodhisattva. At the north gate, place a lotus seat and install the Eight-Armed Avalokiteśvara. To the south, install the Eight Dragon Kings: Nanda-nagaraja (Nanda Dragon King), Vasuki-nagaraja (Vasuki Dragon King), Takshaka-nagaraja (Takshaka Dragon King), Karkotaka-nagaraja (Karkotaka Dragon King), Padma-nagaraja (Padma Dragon King), Mahapadma-nagaraja (Mahapadma Dragon King), Shankhapala-nagaraja (Shankhapala Dragon King), Kulika-nagaraja (Kulika Dragon King). (The eight dragons are only offered polished rice and milk porridge. Other offerings may also be made to the rest.) Light forty-five lamps. First, summon the eight Dragon Kings (using the Hayagriva (Horse-headed) body mudra to come and go).

已。散種種花燒諸名香。沉水薰陸栴檀香等而供養之。咒師一日不食。若不忍饑唯得食蘇。香湯洗浴著新凈衣。入道場中而作供養。常得觀世音菩薩憶念。亦兼通軍荼利法結界供養也。當以真言印護身及結界。咒木七返或咒水七返。又以印咒水。或咒白芥子灰等七返。將木豎四角為界。芥子灰水散於十方間成結界。二手外縛二食指直豎相去五分許。並二大指相著。各屈一節勿著食指。真言曰。

【English Translation】 English version: Thus. Scatter various flowers and burn various famous incenses, such as agarwood, frankincense, sandalwood, etc., for offering. The mantra master should not eat for one day. If one cannot endure hunger, one may only eat yogurt. Bathe with fragrant water, put on clean new clothes, and enter the maṇḍala (道場) to make offerings. In this way, one can often receive the mindfulness of Avalokiteśvara (觀世音菩薩, Bodhisattva's name), and also practice the Kuṇḍali (軍荼利, Vidyārāja's name) method, performing the boundary-establishing offering. One should use mantras and mudras (手印) to protect oneself and establish boundaries. Recite the mantra seven times over wood, or recite the mantra seven times over water. Also, empower the water with the mudra, or empower white mustard seeds or ashes seven times. Place the wood upright at the four corners as a boundary, and scatter the mustard seed ashes and water in the ten directions to form the boundary. The two hands are bound outwards, the two index fingers are upright and separated by about five fen (分, a unit of measurement). 陀波泮底(唐云隨誦成驗)雖不受持壇供養法隨誦成驗。若欲發遣何耶揭唎婆觀世音者。手執素羅毗布瑟波(二合反又云柏花)咒一遍已。一打像上。如是七遍。即歸去也真言曰。

【English Translation】 English version: Dhāraṇīpati (the lord of dhāraṇīs, which was proven efficacious when recited by Yun Gong of the Tang Dynasty) is efficacious even without upholding the altar offering method, merely through recitation. If you wish to send away Hayagrīva Avalokiteśvara (Horse-Headed Avalokiteśvara), hold Surabhi-puṣpa (a type of flower, also called cypress flower) and strike the image once after reciting the mantra once. Repeat this seven times, and He will depart. The mantra is as follows: 『Tadyathā (1) Sara para sara (2) Jaya vijaya (3) Vajra kula sambhava (4) Svapanakakṣa mudutti (6) Svāhā (7)』
